"Eligible Applicants include:(A) Health professions schools, including accredited schools or programs of public health, health administration, preventive medicine, or dental public health or schools providing health management programs;(B) Health Centers such as community health centers, Federally Qualified Health Centers (FQHCs), Academic health centers;(C) State or local governments including state, local and territorial public health departments; or(D) Any other appropriate public or private non-profit entity or tribal entity that trains and/or supports community health workers and health support workers."
