    Description

    The Cooperative Agreement under this ADPI funding opportunity announcement (FOA), will result in twelve, three-year cooperative agreements to Home and Community-Based Service (HCBS) provision organizations to develop or expand the dementia capability of Community Health Worker (CHW) programs, embedding CHW programs in the National Aging Network. The grant program will support development and implementaion of dementia-capable programs to prepare CHWs to support people living with dementia and their caregivers (of all ages) through dementia-specific educational curriculum, training, and services as well as their delivery of dementia-specific programs beyond initiatives, essentially expanding the traditional role of CHWs the communities they serve. The anticipated outcomes of this cooperative agreement include culturally competent training materials and programs resulting in a dementia-capable CHW workforce. Another goal of the program is a focus toward developing a career advancement pathway for a culturally compentent dementia CHW workforce. The award will increase access to meaningful and culturally relevant information, services, and supports for family caregivers, which supports Recommendation 3.1 made by the Recognize, Assist, Include, Support, and Engage (RAISE) Act Family Caregiving Advisory Council in its initial report to Congress.
