Hemophilia, von Willebrand disease (VWD), venous thromboembolism (VTE), and thalassemia are the blood disorders addressed within three separate components in this notice of funding opportunity (NOFO). The purpose of this NOFO is to 1) identify a specific target population that can include individuals diagnosed with a blood disorder (children/adolescents or adults), caregivers, or healthcare providers; 2) promote health knowledge and literacy about blood disorders and treatment among the specific target population; 3) adapt evidence-based self-management program to address the gaps in knowledge, behavioral skills, and the values and barriers to self-management among the specific target population; 4) implement and evaluate the evidence-based self-management program for the specific target population; 5) identify, adapt, and provide innovative evidence-based training for healthcare providers to increase shared decision-making and how to improve medication adherence. These activities should address the long-term outcomes of this NOFO to reduce morbidity and mortality due to blood disorders and improve quality of life.
