    Description

The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $350,000 for Year 1, subject to the availability of funds. This NOFO aims to strengthen sustainable community-led monitoring (CLM) of HIV services in SL. The purpose is to use standardized data collection tools to conduct CLM of HIV services in approximately 96 PEPFAR service sites across 7 districts in SL for dissemination throughout the country to promote improvements in the national HIV response. This NOFO aims to improve HIV care and services for a better client experience, improve health outcomes through community-led site and service monitoring. The objective is to develop and conduct assessments and reporting that reflects clients’ experience in receiving quality HIV services in SL and support stakeholder analysis with actionable insights. Reporting will be done regularly and timely so that real-time pivots can be made to program when gaps are identified. The CLM systems and processes will be in the context of national and community ownership, transparency, and sustainability. Focal areas include HIV prevention and testing services, treatment services (including linkage, retention, and viral load testing), service provider perceptions, attitudes, practices, stigma and discrimination, and client satisfaction with services provided. The recipient will present and analyze data in close collaboration with community representatives, healthcare providers, clients, government stakeholders, and facility managers to ensure dialogue and participation in identifying improvement opportunities and implementing and monitoring solutions.
